Identifying problems in your systems

Changing these systems requires us to identify how culture, structures, policies and practices are holding the problem in place. These are the same forces that will hold solutions in place. The key is shifting the stories we tell ourselves (our “mental models”) that shape culture, structures, policies and practices.
